In Loving Memory of Dr. Terry Ullmann, MD
Dr. Ullmann earned his Medical Degree at University of South Alabama. He completed his Family Medicine residency at Tallahassee Memorial Hospital in Tallahassee, FL.
His experience includes private practice, emergency medicine and urgent care.
Evan Ellenberg, PA-C
Evan completed his undergraduate studies in Neurobiological Sciences at the University of Florida in Gainesville, FL, and attended PA school at South College in Knoxville, TN. He has been in Family Practice for 8 years, and has experience with in office Dermatology and Orthopedic procedures.
Following the loss of his mentor, Dr. Ullmann, Evan took over ownership of the practice.
Tiffany A. Martin, APRN-C
Tiffany completed her undergraduate studies at Clemson University and graduated Summa Cum Laude. She then went on to complete her master’s degree at the University of West Florida. While at the University of West Florida, she was inducted into the Upsilon Kappa Chapter of the Sigma Theta Tau Honor Society. Tiffany is board-certified through the American Academy of Nurse Practitioners.
